What is a confined space inspection: the term, and the duties behind it
A confined space inspection is an examination of a space that is enclosed or largely enclosed and carries a reasonably foreseeable risk of serious injury from the conditions inside it, such as a tank, vessel, silo, sewer, flue, pit or chamber, where entering to look is itself the most dangerous part of the task.
The classification turns on risk, not on size or on how tight the opening is. A large chamber with an oxygen deficient atmosphere is a confined space. A cramped void you can back out of in two seconds, with clean air and nothing that can fill it, may not be.
What counts as a confined space?
In Great Britain the definition sits in the Confined Spaces Regulations 1997, published at legislation.gov.uk: any place, including any chamber, tank, vat, silo, pit, trench, pipe, sewer, flue, well or other similar space in which, by virtue of its enclosed nature, there arises a reasonably foreseeable specified risk.
The phrase doing the work there is specified risk, which the same regulations define as a risk of:
- serious injury from fire or explosion
- loss of consciousness from an increase in body temperature
- loss of consciousness or asphyxiation from gas, fume, vapour or the lack of oxygen
- drowning from a rise in the level of a liquid
- asphyxiation from a free flowing solid, or entrapment by one preventing you reaching breathable air
HSE's plain language version is a space that is both enclosed or largely enclosed and carries a reasonably foreseeable specified risk of fire, explosion, loss of consciousness, asphyxiation or drowning. Note what is absent: nothing about how small it is.
What the law requires, and where it applies
In Great Britain, regulation 4(1) is unusually direct: No person at work shall enter a confined space to carry out work for any purpose unless it is not reasonably practicable to achieve that purpose without such entry. Avoidance is not best practice here. It is the first legal test. Where entry is unavoidable, a safe system of work must cover the specified risks, and regulation 5 requires suitable and sufficient rescue arrangements to be in place before anyone goes in.
These duties are jurisdictional. They are not a global standard. In the United States, confined space work is governed instead by OSHA's own permit required confined spaces standards, which are structured differently. Other countries have their own regimes again. The documents that govern any given job are the local regulator's rules plus the site's own procedure, never a template borrowed from another country.
How a confined space inspection is planned
- Authorisation. Entry is normally controlled by a permit to work, issued only once the isolations and precautions have been verified in place.
- Assessment and sequence. A risk assessment for the space and its contents, and a method statement setting out the steps, in order.
- Atmosphere. Tested from outside before entry and monitored continuously during it, for oxygen, flammables and toxics relevant to that space.
- Isolation and ventilation. Lines blanked or locked off, residues cleaned, forced ventilation running.
- People outside. A trained attendant at the entry maintaining communication and the entry log. Confined space entry is the textbook opposite of lone working.
- Rescue. Equipment ready and a trained team briefed. Calling the emergency services is not a rescue plan.
Why rescuers are so often the casualties
HSE describes an incident in a crane compartment where water ingress and rusting had consumed the oxygen. Three men were extracted by the emergency services and two of them died, and the casualties included men who had gone in to help the first workers. That pattern repeats across incident reports: the atmosphere is invisible, the collapse is instant, and the instinct of a colleague standing at the opening is to climb in. Everything about a confined space procedure is built to interrupt that instinct.
Confined space inspection in practice: a worked example
A below ground valve chamber on a water treatment site is due a condition inspection covering the pipework, the chamber walls and the fixed ladder. The team lifts the cover, tests the atmosphere on a probe from the surface, and photographs the pipework and walls on a pole camera without anyone descending. Two of the three questions are answered from ground level. Only the ladder fixings need a hand on them, so a single permitted entry with a tripod, harness, gas monitor and attendant replaces what used to be a routine descent for the whole job.
Can the entry be avoided altogether?
This is the question the regulations ask first, and the honest answer is often a partial yes. Pole cameras, borescopes, drop cameras, fixed sensors and structured photo and video capture by whoever is already on site can settle a surprising number of questions from outside the opening: has the coating failed, is the level right, is there debris, has anything changed since last time.

Two things it does not do, and no camera does. It does not remove a statutory duty: if entry is still necessary, every requirement above applies in full. And it does not replace the judgement of a competent person, who still decides what the evidence means. What it changes is how many entries are needed to reach that decision, and given what a confined space does to people who enter it, an avoided entry is the single largest safety gain available in this whole area.
